Some tools and machinery might give you a little pinch or a graze or small cut if you use them incorrectly but there are others that will see your end if you aren't careful and you fall into complacency.  Here's a list of factors (in no particular order) that can contribute to safety problems when using equipment

- No training
- Safety guards not installed or not used
- "Horseplay" and mucking around
- Poor maintenance of equipment
- No PPE or incorrect use of PPE
- Hazard not being reported or not being followed up
- Cluttered workspace
- Incorrect use, wrong tool for the job
- Not checking the work area for hazards before starting
- Rushing, working under stress
- Using equipment while under influence of drugs or alcohol
- Forcing equipment instead of letting it do its job
- Using tools not matched to the stature of the user
- Use around water / wet surfaces
